Job Description – Home Hemodialysis or HHD Travel RN (Registered Nurse)
Provide in-home training and clinical support to patients and caregivers learning to perform dialysis independently in a home setting.
Assess patients’ physical and emotional readiness for home dialysis, including suitability of the home environment.
Deliver patient education and hands-on instruction covering dialysis procedures, infection prevention, machine operation, and emergency protocols.
Evaluate patient competency throughout training and provide ongoing support post-training through telehealth or in-home visits.
Collaborate with nephrologists, home program coordinators, dietitians, and social workers to build individualized treatment plans and monitor patient progress.
Document training activities, patient assessments, and ongoing clinical status in the electronic medical record (EMR).
Perform equipment setup, calibration, maintenance, and troubleshooting for home dialysis machines.
Monitor patient adherence and intervene promptly if complications, concerns, or non-compliance arise.
Ensure compliance with all safety, infection control, and quality assurance standards related to home hemodialysis.
Provide emotional support to patients and families managing chronic kidney disease in the home care setting.
Participate in continuous quality improvement (CQI) and program development initiatives.
Job Requirements
1+ year of recent experience as a home hemodialysis (HHD) RN
Graduate from an accredited nursing school
Current state specific nursing license in good standing, or if applicable, a compact state RN license in good standing
BLS certification required (Chronic, HHD, and Acute settings)
